Financial Policy/Insurance Information. We made every effort to provide you with accuarte information regarding your insurance and financial responsibility. 

Patient Responsibility/Insurance. We will file your insurance claims on your behalf, and while we may have a predetermination fro your services, that is not a guarantee of payment by the insurance company. The portion estimated to be your responsibility will be due on the day of service. 

If we have no insurance information on file, payment is due, in full, on the day of service.

It is your responsibility to know your plan benefits and to pay any deductible, co-insurance or balance not paid by your insurance company. 

Estimates. An estimate of charges will be given to you upon your initial consultation appointment. We will file a predetermination with your insurance company if time allows. Please note that it takes 2-4 weeks for an insurance company to process a predetermination and is not a guarantee of payment. 

Payment Methods.  We offer several options for payment including cash, check, Visa, Mastercard, American Express and Discover. We also offer convenient interest free financing through CareCredit. Information can be found at www.carecredit.com

Billing Statements. Once your insurance benefits have been applied, you will receive a statement from our office with any remaining balance. Any remaining balance after the insurance has made payment is your responsibility. Payment for any outstanding balance must be received within 30 days. 

Refunds. Any overpayment will be refunded to the appropriate parties. No refunds will be issued until all insurance has been processed and paid. Refunds less than $5.00 will not be issued unless requested.
